Government of Punjab Primary and Secondary Healthcare Department Punjab Health Facilities Management Company PHFMC Jobs 2020 Application Form Walk in Interview. i. The name of the post applied for should be mentioned on top of the envelope.
ii. Applicants should attach two sets of attested copies of all relevant Educational documents, CNIC, Passport size photographs and proof of relevant work experience.

iv. Submit the Application at the concerned District Offices as given in the Advertisement.
